Blood Glucose Levels: A Guide

Maintaining healthy/optimal blood glucose levels is important in living with diabetes and managing your condition [1, 2]. 

Monitoring and maintaining stable glucose levels in the blood and other lifestyle changes, such as eating healthily, engaging in physical activity, and maintaining a healthy weight, are key for diabetes care and quality of life [1, 2].

What are blood glucose levels? 

Blood glucose is the main source of energy for the cells in your body [2, 3]. Blood glucose is created when carbohydrates from your diet are metabolised [3]. Glucose is then used immediately or stored in the liver and muscles for use later [3].

When you’ve had food, it gets digested into the gut and absorbed into the intestine, which increases blood glucose levels [2]. When fasting, blood glucose levels drop, and the liver helps keep the glucose in the body healthy by making its glucose [2]. In both scenarios, the pancreas needs to produce the hormone insulin, which is necessary for glucose to enter the cells so that the cells can turn glucose into energy [2].

In diabetes, there is either an insufficient production of insulin (type 1 diabetes) or increased resistance to insulin (type 2 diabetes) [4] — in both situations, this interferes with the ability to maintain healthy blood glucose levels. Therefore, understanding and monitoring blood glucose level ranges is a key part of managing all types of diabetes.

 

Why is it important to maintain blood glucose levels?

For a person living with diabetes, there is a risk of complications. These complications can be divided into microvascular and macrovascular [4].

Research shows that keeping blood glucose levels in check can reduce the risk of microvascular complications — conditions affecting small blood vessels [1, 4, 5]. 

Microvascular complications include:

  • diabetic retinopathy, or complications that damage the eyes and may even lead to blindness [4]
  • diabetic neuropathy, or complications that damage the nervous system, such as diabetic foot, which may lead to amputation [4]
  • diabetic nephropathy, or conditions that affect the kidneys and renal system, such as chronic kidney disease [4]

Macrovascular complications include:

  • cardiovascular disease [4]
  • stroke [4]
  • peripheral artery disease, which may lead to bruises or injuries that do not heal, gangrene, and ultimately, amputation [4].

Good glycaemiccontrol may also help to reduce the risk of some of these complications, such as heart attacks [5], [1, 4].

However, a combined effort to keep blood glucose, cholesterol, and blood pressure levels in check-in diabetes can lead to more favourable outcomes and minimise the risk of diabetes-related complications overall [4].

 

High blood glucose levels & hyperglycaemia 

If someone has a blood glucose reading over 7.8 mmol/L, they are considered to have high blood glucose or hyperglycaemia by medical terms [6].

Numerous factors can cause hyperglycaemia, but in diabetes, it occurs when insufficient insulin or insulin is not used properly [6, 7].

Hyperglycaemia can cause symptoms such as frequent urination, frequent thirst and fatigue, blurred vision, and recurrent infections [1].

Untreated hyperglycaemia can increase the risk of complications, some of which are life-threatening. 

These include heart disease, stroke, peripheral vascular disease, kidney and eye disease, metabolic syndrome and neuropathy [7].

 

Low blood glucose levels & hypoglycaemia 

Hypoglycaemia, or low blood glucose, is medically defined as lower than 3.9 mmol/L [8, 9, 10].

In diabetes, it can occur as a side effect of taking insulin or other insulin-increasing drugs (e.g. sulfonylureas) [9, 11]. It can also be caused by not consuming enough carbs, fasting, increasing physical activity, drinking too much alcohol without enough food, or being sick [9]. Severe liver or kidney disease can also increase your risk for hypoglycaemia [11].

Symptoms of hypoglycaemia include feeling shaky, tired, hungry, irritable or confused, dizzy, or lightheaded [9]. Additionally, you may have a rapid heartbeat and not feel able to think or speak clearly [9].

Severe hypoglycaemia may lead to seizures, loss of consciousness, coma, or even death [9]. Other long-term and short-term complications include hypoglycaemia unawareness, neurologic damage, trauma, and cardiovascular events such as angina and heart attack [9].

 

Normal blood glucose levels

The World Health Organization (WHO) [10] and the American Diabetes Association (ADA) [12] define blood glucose levels for adults aged 18 and older as:

  • normal blood glucose levels when fasting (i.e. before breakfast or without having eaten anything in the previous 8 hours) are between 3.9 mmol/L and 5.6 mmol/L 
  • fasting blood glucose between 5.6 and 6.9 mmol/L indicates that changes in lifestyle and monitoring blood glucose levels are necessary. According to the ADA, this constitutes prediabetes 
  • fasting blood glucose of 7 mmol/L or higher on two separate tests suggests a diagnosis of diabetes 
  • I would add our SEMDSA guideline numbers here and A1c as well. Need to look it up

Blood glucose targets can vary from individual to individual, based on several factors such as [13]:

  • duration of diabetes
  • age or life expectancy
  • conditions a person may have
  • cardiovascular disease
  • other diabetes-related complications
  • hypoglycaemia unawareness
  • other individual patient considerations

For children and adolescents with type 1 diabetes, recommended fasting blood glucose levels are 5.07.2 mmol/L [14]. A1C levels should be lower than 7.5% (58 mmol/mol), and after-meal blood glucose levels should be measured when there is a discrepancy between fasting blood glucose values and A1C levels [14]. 

It is important to remember that the above are just guidelines. A core element of blood glucose control is that targets should be individualised based on a benefit-risk analysis carried out by you and your healthcare team [1, 5, 14].

 

Testing blood glucose levels

There are several ways in which you can test your blood glucose levels. You can do so yourself or your healthcare team may recommend specialised tests. 

Self-monitoring can be done using either [8, 9, 12, 14]: 

  • A blood glucose meter — essentially a finger-prick test, also known as finger-stick test
  • Continuous glucose monitoring systems (CGMs)
  • Flash glucose monitoring systems (sometimes known as ‘intermittently scanned continuous glucose monitors’)

Your healthcare team may also test your blood glucose levels, especially to diagnose diabetes. These tests include [12, 15]:

  • the HbA1C test, also known as the glycated haemoglobin test 
  • the fasting plasma glucose (FPG) test
  • the random plasma glucose (RPG) test
  • the oral glucose tolerance test (OGTT) [12]

Testing blood glucose levels is essential for diagnosing diabetes promptly, managing the condition, and prevent potentially life-threatening complications [15].

Maintaining healthy blood glucose levels is a core element of diabetes care and self-management. Monitoring glucose levels in the blood through rigorous testing may help reduce the risk of  micro- and macrovascular complications, prevent hypo and hyperglycaemia, and maintain a good quality of life for those living with diabetes.
